Whistleblower Tells of America's Hidden Health Nightmare for its Sick Poor

    When an insurance firm boss saw a field hospital for the poor in Virginia, he knew he had to speak out. Here, he tells Paul Harris of his fears for Obama's bid to bring about radical change.

    Wendell Potter can remember exactly when he took the first steps on his journey to becoming a whistleblower and turning against one of the most powerful industries in America.

    It was July 2007 and Potter, a senior executive at giant US healthcare firm Cigna, was visiting relatives in the poverty-ridden mountain districts of northeast Tennessee. He saw an advert in a local paper for a touring free medical clinic at a fairground just across the state border in Wise County, Virginia.

    Potter, who had worked at Cigna for 15 years, decided to check it out. What he saw appalled him. Hundreds of desperate people, most without any medical insurance, descended on the clinic from out of the hills. People queued in long lines to have the most basic medical procedures carried out free of charge. Some had driven more than 200 miles from Georgia. Many were treated in the open air. Potter took pictures of patients lying on trolleys on rain-soaked pavements.

    For Potter it was a dreadful realisation that healthcare in America had failed millions of poor, sick people and that he, and the industry he worked for, did not care about the human cost of their relentless search for profits. "It was over-powering. It was just more than I could possibly have imagined could be happening in America," he told the Observer

    Potter resigned shortly afterwards. Last month he testified in Congress, becoming one of the few industry executives to admit that what its critics say is true: healthcare insurance firms push up costs, buy politicians and refuse to pay out when many patients actually get sick. In chilling words he told a Senate committee: "I worked as a senior executive at health insurance companies and I saw how they confuse their customers and dump the sick: all so they can satisfy their Wall Street investors."

    Potter's claims are at the centre of the biggest political crisis of Barack Obama's young presidency. Obama, faced with 47 million Americans without health insurance, has put reforming the system at the top of his agenda. If he succeeds, he will have pushed through one of the greatest changes to domestic policy of any president. If he fails, his presidency could be broken before it is even a year old. Last week, in a sign of how high the stakes are, he addressed the nation in a live TV news conference. It is the sort of event usually reserved for a moment of deep national crisis, such as a terrorist attack. But Obama wanted to talk about healthcare. "This is about every family, every business and every taxpayer who continues to shoulder the burden of a problem that Washington has failed to solve for decades," he told the nation.

    Obama's plans are now mired and the opponents of reform are winning. The Republican attack machine has cranked into gear, labelling reform as "socialist" and warning ordinary Americans that government bureaucrats, not doctors, will choose their medicines. The bill's opponents say the huge cost can only be paid by massive tax increases on ordinary Americans and that others will have their current healthcare plans taken away. Many centrist Democratic congressmen, wary of their conservative voters, are wavering. The legislation has failed to meet Obama's August deadline and is now delayed until after the summer recess. Many fear that this loss of momentum could kill it altogether.

    To Potter that is no surprise. He has seen all this before. In his long years with Cigna he rose to be the company's top PR executive. He had an eagle-eye view of the industry's tactics of scuppering political efforts to get it to reform. "This is a very wealthy industry and they use PR very effectively. They manipulate public opinion and the news media and they have built up these relationships with all these politicians through campaign contributions," Potter said.

    Potter was witness to the campaign against Michael Moore's healthcare documentary Sicko. The industry slammed the film as one-sided and politically motivated. Secret documents leaked from the American Health Insurance Plans, the industry's lobby group, detailed the plan to paint Moore as a fringe radical. Potter now says the film "hit the nail on the head". "The Michael Moore movie that I saw was full of truth," he admits.

    Potter was also working for Cigna when it became embroiled in the case of Nataline Sarkisyan, whose family went public after Cigna refused to pay for a liver transplant that it considered "experimental" and therefore not covered by their policy. Cigna reversed this decision only hours before the Californian teenager died. "I wish I could have done more in that case," Potter said.

    Such sentiments are rare in an industry that has given America a healthcare system that can be cripplingly expensive for patients, but that does not produce a healthier population. The industry is often accused of wriggling out of claims. Firms comb medical records for any technicality that will allow them to refuse to pay. In one recently publicised example, a retired nurse from Texas discovered she had breast cancer. Yet her policy was cancelled because her insurers found she had previously had treatment for acne, which the dermatologist had mistakenly noted as pre-cancerous. They decreed she had misinformed them about her medical history and her double mastectomy was cancelled just three days before the operation.

    Last month three healthcare executives were grilled about such "rescinding" tactics by a congressional subcommittee. When asked if they would abandon them except in cases of deliberately proven fraud, each executive replied simply: "No."

    To Potter that attitude has a sad logic. The healthcare industry generates enormous profits and its top executives have a lavish corporate lifestyle that he once shared. Treating patients for their expensive conditions is bad for business as it reduces the bottom line. Kicking out patients who pursue claims makes perfect economic sense. "It is a system that is rigged against the policyholder," Potter said. The congressional probe found that just three firms had rescinded more than 20,000 policyholders between 2003 and 2007, saving hundreds of millions. "That's a lot of money that will now go towards their profits," Potter said.

    A lot of that money also goes into contributions to politicians of both parties - $372m in the past nine years - and in lobbying groups to run TV ads slamming Obama's plans. Many of these ads deploy naked scare tactics. One report said that the industry was spending $1.4m a day on its campaign. In the face of that, it is perhaps no wonder that the Senate has delayed its vote, dealing a massive blow to Obama. "I have seen how the opponents of healthcare reform go to work... they are trying to delay action. They know that if they keep the process going for months, and turn it into a big mess, then the political impetus behind it will lessen," Potter said.

    Potter, who now works at the Centre for Media and Democracy in Wisconsin, says the industry is afraid of Obama's reforms and that is why it is fighting so hard. It wants to deal him the same blow as it did Bill Clinton when it scuppered his attempt at reform in the 1990s. Potter admits that he is worried the industry might win again. "I have seen their tactics work. I have been a part of it," he said. He knows he has no chance of ever working again for a major firm. "I am a whistleblower and corporate America does not tend to like that," he said. But there is one thing Potter is not sorry about: leaving the healthcare industry and speaking out. "I have absolutely no regrets. I am doing the right thing," he said.

    Comprehensive healthcare reform in the US has been an ambition of many presidents since the early part of the 20th century. None has succeeded in creating a system that gives all Americans the right to coverage. Barack Obama, below, is desperate to avoid the same fate.

    Finding a Cure

    It is a complex mish-mash of systems. Millions of Americans have their own private healthcare plans, either individually or through their employer. About 47 million Americans have none. However, systems do exist to cover the very poor and the old. The system is fiendishly complex and full of loopholes, so even those with coverage can have it withdrawn.

    How Bad is It?

    US hospitals are the best in the world if you can afford them. Many cannot, and an accident or sudden illness can often bankrupt someone.

    How Does it Compare With Other Countries?

    It depends how you measure things. The US spends about 16% of GNP on healthcare, far more than France and Germany, which spend 11 to 12%. Yet those countries provide universal care.

    What is the Biggest Problem?

    Critics say the biggest issue is the profit motive that drives US healthcare. This ensures that costs are always rising as the incentive is there to provide expensive treatment. It also gives health insurers the incentive to refuse treatment to claimants, by seeking to withdraw their cover.

    What is Obama's Solution?    Obama has asked Congress to draw up a government option, allowing all Americans to get some sort of cover. The sheer size of the state plan should theoretically allow it to drive down costs by economies of scale.

    What's Happening Now?

    Obama has put his reputation on the line to persuade wavering Democrats and moderate Republicans to vote on legislation by August. The Senate has said this will not happen. That's a major blow, as it puts off the debate until September and could see the political momentum stall.

Call me naive but this delay

Wendell Potter: you are a courageous man brimming with integrity & a sense of compassion many in your industry go out of their way to prevent themselves from acquiring. As a lifelong advocate of healthcare reform, I can only say that I wish there were a thousand more like you because it may take that for our country to actually GET reform of any kind. I'm glad Mr. Potter was

I'm glad Mr. Potter was honest enough to be a whistle-blower against one of the greediest and most heartless segments of US industry. We need to compile the industry contributions to our lawmakers and publish them. This obscene buying of our government should not be allowed to prevent us from getting to the most reasonable solution: "Medicare for all." This would save money, provide for choice, compensate providers fairly, and provide health care (starting with prevention, instead of expensive tertiary care).

I'm fast begining to think that Wall street is the root of most of the problems facing America and it's sheepish followers. Fix Wall Street and the majority of problems disapear, Only then will the people start to regain their status in the financial scheme of things. The only purpose to the delay a health care vote is to allow more time for fear-mongering, thus weakening a proposed national health care plan. I used to think that the Democrats (led by Harry Reid) were uncomfortable being out of their underdog role. They've had enough time now. It is obvious the Democrats who are doing the heel-dragging are not representing their constituents. Along with the Republicans, they are representing the corporate money. Thank you Mr. Potter for having the guts to testify what you know from your unique position. It is the moral and right thing to do, and for those with enough actuarial brain cells to think beyond the next fiscal quarter, it could be cost-effective for generations to come.

It is a terrible mystery to the rest of the world watching the citizens of the land of the free, get shafted and lied to by its leaders and its health care providers. I am a fifty year old Canadian. You must believe us and spread the word, you are all being lied to as Mr Potter says.Not just about your own coverage but about the reality of other world systems. In my family, we have experienced several serious illnesses including cancer, but we have never been contolled in our choice of doctors, we have never been refused treatment, although occasionally we have waited.And we have never been forced to write a cheque to a coorporation for coverage. The fundamental nature of healthcare comes from compassion, but in the US it has been highjacked by the profit motive. When are americans going to wake up and realize that the bogeyman under their (hospital) bed is real. But its not socialism, it is the parasitical HMO industry and the compliant senators and congressmen who do their bidding.

Reality has been that the uninformed have drowned out the informed by responding to the tactics of FUD (Fear Uncertainty, Doubt) used by the opponents. I call this the Elmer plan and it has worked exceedingly well because the American people are easily manipulated due to apathy and ignorance. If you want to see how ignorant the average American is, tell your neighbor that the US has the worst health care system in the world when it comes to combating preventable deaths. Your neighbors will all argue with you and call you a traitor for spouting such nonsense even though it is provably true. If everybody who reads this article convinces one person that what the article says is true, then there will be hope. If, instead,, 995% of the readers of this article simply say "So, I have my coverage and don't want to get involved." Then nothing will change and the richest nation in the world will continue to have the worst healthcare system. When somebody accuses the reform as involving rationing, explain to them that rationing of health care exists today as follows 1. The rich get the best in the world. 2. The very poor get mediocre through Medicaid. and 3. Those in between get the best that they can afford.

What's also amazing about this story is Mr. Potter's observations of the clinic. Most of us that read these articles never see that side of our health care system. Interestingly, people I work with are LIVID that their health insurance benefits may be taxed. Honestly, it's frightening how upset they become. When I ask, would you sacrifice $100 a month so that everyone could be covered, especially since you pay for that and more anyway. They still say no. We live in an unforgiving and ignorant world...

Another aspect that will clearly play out in this controversy is the "information wars." One wonders why a documentary is not being done and shown nationally to bring into the public eye exactly what Potter saw years ago and that is going on at a larger scale right now. The answer is that the media are in the same mentality and in the same pockets as the Healthcare industry. Health Industry [misnomer] lobbyist are working day and night to keep the media from publicly exposing the harsh reality that their policies and actions have created and exacerbated. Instead, we will see a continuing barrage of ads and "public service" announcements criticizing any and every reform measure, Let's face it, any reform measure would improve health care access and delivery and is likely to reduce the profit margins of the companies. It is not enough to make a reasonable profit. They need to make excessive profit, and their tactics regarding insureds, the uninsured and the poor are designed to increase those profits. Short term gain and to heck with ethics or any semblance of humanity.

I feel the root of most

I feel the root of most problems in America, including healthcare, is the concept of "Corporate Personhood". As long as corporations, who have millions or billions of dollars to fund their forms of free speech (i.e. campaign contributions), have the same rights as ordinary citizens, nothing will change. Unfortunately, revoking corporate personhood would likely require a constitutional amendment.

Thank you, thank you Mr. Potter. I spent 13 year embroiled with the system until my husband, who was a NASA engineer, died in 'o4 from chronic progressive multiple sclerosis. As I was his only caregiver, because, of course, caregivers are not provided for under most policies, I now must deal with the loss of the man I truly loved and post traumatic stress syndrome, along with chronic depression problems that I already had and oh, let's not forget that all of our money is gone so I will have to work until I die. At least I have medical care because I have immigrated to Canada. The system here is great and no insurance companies should be allowed to lie in commercials and say otherwise. Yes, there is triage, but there should be and yes, people are expected to take some care of their own health, but there is a lot of help for that and good education. The situation in the U.S. is a travesty and I cannot see that reform is possible until the insurance companies are gone.

If we can use this delay to rally forces and pressure our representatives, that will be a good thing. Problem is twofold: I don't see an organizing group that will get people out -- away from their computers and into the streets. Perhaps the California Nurses can do it. But we need to be organized and mobilized. I understand there is a July 30 rally in DC. Problem is, the date is too soon. People have made summer plans. We need to show resolve (even if we disagree on details) and get in the face of our government.

It is time for We The People to let our politicians know that we will no longer stand behind them if they don't stand behind us. This is why we have term limits, so we can oust those rascals who only line their pockets with no thought to whom they may be harming along the way. Call your Senator. Write to your Senator. Just to show you how crooked the private health insurance industry is take any medical device, say vacuum erection pumps used by persons with diabetes to enjoy se with an erection. While Medicare pays 100% with a doctor's prescription even on deluxe models, Blue Cross Blue Shield will only pay for manual models rather than battery operated ones at the identical price. They also insist on buying from in-State suppliers even at a 70% surcharge. Think about it, not just the denial of patient wants, but the determination of who gets paid. One can only suppose that there is some kickback arrangement. So I have heard the industry claims about the de-personalization of care under socialized medicine, but let me tell you first hand, the current system feels more like some faceless bureaucrat from private insurance is telling me just how I am able to take life's most intimate pleasures. I wonder if ths isn't the same mentality that used to reject pain killers for cancer victims. GEt the Blue Cross/Blue Shiled, Cigna and etc. bureaucrats out of our private lives and give us a responsive public plan that is not profit-based.

How long will the American

How long will the American Citizen put up with the oppression of big business' lust for unreasonable profits. If President Obama's health care reform fails or is watered down too far, the "silent majority" should take up arms against their corporate and political oppressors. I realize that last sentence is pretty radical, but the status quo is the oppression of the middle class and the poor. Universal health care should actually make for a healthier work force and therefore less absenteeism followed by a healthier bottom line for most businesses. To those who fear a raise in taxes, would you consider giving up say $600.00 a month in health insurance premiums for $200.00 per month in increased taxes a fair trade? It is a travesty to think that government can effectively run a nationalized healthcare system. Nowhere in the world is this done efficiently and cost effectively. We need to expand coveraqe to the uninsured but keep the quality and efficiencies of a privatized system. I grew up in England..... please believe me that we do NOT want the government to run our healthcare system!!!! It would be a true money sink, and drastically decrease access to treatment, and quality of treatment. (try getting life saving kidney dialysis in the UK if you are over 65....how about a 6 month wait to see a pulmonary specialist)

In reply to "orange Julius" - People are outraged about taxation of benefits becasue the tax is very regressive. Payroll tax alone, before income tax, will add $1,835 to the average $12,000 per year family policy (15.3% employer-employee tax). This hurts both workers and businesses. Investors will avoid the payroll tax, paying a maximum of 15% dividend/cap gains taxes. We need to CUT the cost of healthcare, no add to the problem via the tax code!

What a powerful article. The time has come for reform. I think it will happen. Look at all the people who said Obama wouldn't be president and yet today he is. We just have to keep the pressure on. There is a website, opencongress.org I believe, that tracks the activity of our congresspeople and I think also lays out the percentages of campaign contributions from various industries to each member's campaign. A very powerful tool!

Please don't fall for the

Please don't fall for the "competitive with market" Public Option BS. It is a farce by a President and a Party who want to seem successful. If they gets this thing passed with a "Public Option" they will be seen as successful. But the Public Option will fail to do anything but set us back to where we are now (which is why the AMA/insurance companies have backed down recently). Why? Simple, it doesn't automatically enroll anyone, as Medicare did automatically. I just have to wonder, what about the doctors whose patients are ready for their procedures and are canceled by their insurance companies? Are they not obligated to continue treating their patient? How can a doctor, who has take the Hippocratic oath just let them walk away untreated, just because the insurance company revoked their policy? How come that doesn't scare the bejesus out of people? Those who are against a public option seem to be buying into the lie that the government will come between them and their doctor, but the insurance companies are already doing that, and for a PROFIT no less, something the government option will not d0. Also, the public option is for insurance, not for government run hospitals etc.

I hope that each and every person who reads this takes an hour out of this week to let them know that they, the common citizen are the majority and not the corporate executives. But you must write your representatives frequently, reiterating that: 1) There really is no "Wall Street" or "Insurance Companies" or "Banks" - not one of those institutions can vote, only the corporate executives themselves can vote. You are more, they are the few. We are a government "OF the people, BY the people, FOR the people" - you my friends, are the people. 2) Health must be universal and health insurance companies must accept all takers (that is the whole idea behind risk) or get out of the business. 3) That employer health plans must be decoupled from ERISA so that employee patients have legal recourse (you currently cannot sue employer-sponsored plans for more than the costs of procedures, you must pay your lawyer yourself from any proceeds) 4)Healthcare is a right without which we cannot expect to have life, liberty and the pursuit of happiness. Sick people are a drain on American productivity. It's hard to be a pioneer, do your best, invent a new technology or be pleasant to a customer if you or your family member can see no way out of their physical or mental predicament. 5) We have mandatory education yet private schools thrive. There is no reason to believe that private insurance cannot co-exist with gov't insurance 6) Medicare administrators companies are are not run by the government, they are public & private companies that compete for the government's business. There are plenty of opportunities for businesses under universal healthcare. Write them today, write them frequently. So we spend 5% more on healthcare than England and France who both cover everybody, even you if you were a migrant in France. So it looks like we could spend 5% less of GDP if we went to Universal Health Care and covered everybody including Mexican illegals.

There is a group, Public

There is a group, Public Citizen, or citizen.org, that works tirelessly for health care, campaign finance and energy reform. Moveon.org is another large group of real people (not inhumane corporate monopolies) that work for people. Join both and suggest they get together. We need a 5 million man march on DC! Everyone needs to see the documentary, The Corporation, which does a psych eval on a corporation as though it was a person.....diagnosis....Psychopath. Lets take action to remove this sickness from our midst.

From Jim Lerher NewsHour and NPR to ABC/NBC/CBS to CNN and of course FoxViews, they have all gone out of their way to distort facts and fabricate lies and omit truths in order to defeat this cause. Obama is letting us ALL down. The piece by Kip Sullivan: Bait & Switch told us how the ORIGINAL public plan has been broken down to the loser plan they are hashing out now. We are going to end up with a mandated PRIVATE "option" that will kill us all. Single-payer, the BEST plan, was silenced by design. I hold the media hacks all responsible for this travesty.

I am a 75 year old Canadian

I am a 75 year old Canadian and my wife is 73. I pay $1,100 a year for full coverage. We can enter any of dozens of walk-in clinics, just show our national insurance card and be treated and just walk out. No bill. Go to the hospital: no charge. Visit your own choice of physician or surgeon: no charge. We all just walk in and walk out: everybody gets the best possible care and there is one single payer which is the national health plan, and we the people of Canada have empowered them to operate a decent social contract that is based on compassion. Human health is a human RIGHT, not a privilege. And we are NOT commies or pointy-headed ''liberals'' as you call us. Personally I am appalled by the horror of the US system. The cruelty and greed of the US ''system" is a sign of a deep sickness in your society. I am afraid that you are collapsing. Think of your children for God's sake !

The congressmen in both houses who are fighting healthcare reform in general and the public option in particular deserve no respect! I mean zero respect at all! Bill Moyers did a super fine interview with Mr. Potter on a recent (last Friday?) broadcast on PBS. Highly recommended.

If Wendell Potter is a great-grandson of the horrible Potter of "It's a Wonderful Life" I guess he has rectified the mean reputation and will be insured entry into heaven. The interview with Bill Moyers was fantastic and I was in tears a couple of times as Mr. Potter spoke. I was so touched my his new found humanity. In September, lets all schedule a HUGE rally in front of Congress and the insurance companies. WE HAVE TO FIGHT THIS WITH ALL WE'VE GOT.

This episode by Wendell Potter really has to be mainstream information. Send it to whomever you can, publicize it. My father was a physician who began his career in 1950. Before he retired in 1990 he was already feeling a lot of unhappiness with the medical system, which he said was no longer keeping the welfare of the patient as the priority. As this a man in Canada said (quoted earlier): "...there is one single payer which is the national health plan, and we the people of Canada have empowered them to operate a decent social contract that is based on compassion. Human health is a human RIGHT, not a privilege." We need to as a country get back on track with our moral inspiration, not fanatically, just simply look inside and see what needs to be done, and just simply do it and not cease or settle for less than the best. If we look to our own personal health and do all the natural things that are best for us, we are more calm and in a position to represent what we want to see nationwide. That's BEING the solution.

Last week while in DC at a health care rally, I had the opportunity to talk with a Congressman at great length about health care reform and where everything was headed. When asked if the Congress is seizing upon Mr. Potter's testimony, the Congressman said, "yeah, the White Hose people are talking to him." Talking to him! They should be using this guy as a front man for reform. They should be telling the American people what this guy is telling them. They should escort him around to every legislative committee and demand those legislators listen to what he has to say about an industry that has done more to harm the American people than 100 Osama bin Ladens.
